Job description

Position Summary

V-Tech Solutions, Inc., is seeking motivated candidates to serve as Community Health Workers in Washington, DC to support our client. The Community Health Worker will promote relationships with beneficiaries and families of beneficiaries to local and federal assistance programs, working collaboratively with service providers and organizations to improve access to health and wellness resources. CHW’s will work Tuesday-Saturday 11am-7:30pm.

Responsibilities

· Conduct Daily Home visits to pre-identified Medicaid beneficiaries

· Listen to Connect community members to healthcare services needed.

· Provide health education as necessary

· Schedule wellness, dental, and lead appointments for enrollees

· Effectively, establish rapport, and show compassion to enrollees.

· Identify and promote access to assistance programs for food, vaccines, case management, shelter etc.

· Support community engagement strategies and maintain records of outreach activities.

· Maintain confidentiality of enrollee information.

· Work independently as well as support teamwork.

Requirements:

· Education: High School Diploma or GED (associate or bachelor's degree in public health, Social Work, or related field preferred).

· Valid Driver’s License

· A personal vehicle for travel – mileage reimbursement is available

Certifications: Public health, Wellness, or peer to peer support (If available)

Skills & Abilities:

Strong Interpersonal and communication skills Ability to engage diverse communities and build trust

Knowledge of local and federal programs

Experience working with community organizations and service providers

Ability to identify access points and promote available resources

Organizational skills and ability to manage outreach activities

Basic Data Entry Intermidiate knowledge of excel Adept time management
