POSITION SUMMARY: Pilsen Medical Clinic & Associates is actively seeking Independent Contractor (APNP) Advanced Practice Nurse Practitioner to complete home visits around the Chicagoland area. We invite you to choose a career with our growing home visiting physician team! As a team we work together to provide primary care services for seniors in their homes. Our goal is to provide seniors with the highest quality medical services in the greater Chicago area, emphasizing wellness and management of chronic diseases. Each practitioner is partnered with a medical assistant to ensure exceptional, quality care for each patient.

  • Coordinate patient care with patient's associated home health agency
  • Review patient case document in an timely manner.
