Description:
The Trinity Health PACE Chaplain serves as a clinical spiritual care provider who supports patients, participants, families, and staff by addressing spiritual distress, grief, crisis situations, and major life transitions. As an integrated member of the interdisciplinary healthcare team, the chaplain assesses spiritual needs, delivers individualized care aligned with diverse beliefs, documents interventions in the medical record, and contributes to holistic care planning. The role also includes developing and improving spiritual care programs, educating staff, promoting workplace spirituality, facilitating religious practices in collaboration with community faith leaders, and maintaining sacred spaces. Additionally, the chaplain is expected to uphold ethical and professional standards, engage in ongoing self-care and formation, and possess the necessary education, clinical training, and communication skills to effectively meet the spiritual needs of the community served.
Key Responsibilities
Spiritual Care Delivery
  • Provide compassionate, individualized spiritual care to patients, families, and staff.
  • Facilitate support during illness, crisis, grief, and end-of-life situations.
  • Ensure respect for diverse religious and cultural beliefs within care.

Assessment and Documentation

  • Educate staff on recognizing and responding to spiritual needs.
  • Promote workplace spirituality and staff well-being initiatives.

  • Facilitate prayer, worship, sacraments, and meditation opportunities.
  • Collaborate with community clergy and faith leaders.

Professional Practice

Minimum Qualifications
  • Master's degree in theology, Religion, Spiritual Care, or related field (or equivalent experience).
  • 1-3 years of spiritual care experience, including Clinical Pastoral Education (CPE).
  • Board Certified Chaplain (preferred).
  • Strong interpersonal, communication, and clinical documentation skills.
  • Ability to work collaboratively in a multidisciplinary environment.
  • Proficiency with electronic documentation systems and standard software tools.
