We are a newly established hospice organization committed to delivering compassionate, dignified, and personalized end-of-life care to patients and their families in the Milwaukee area. As we grow, we're seeking a dedicated PRN Chaplain to join our clinical team and help shape the future of our care model.


Position Summary:
We are seeking a compassionate, experienced Hospice Chaplain to join our interdisciplinary team. The Chaplain provides spiritual care and emotional support to patients, their loved ones, and staff. This role is essential in addressing the spiritual and emotional aspects of end-of-life care, supporting the interdisciplinary team, and ensuring compliance with hospice regulatory standards, including those of the Centers for Medicare & Medicaid Services (CMS) and Wisconsin Administrative Code DHS 131.29.

Requirements:Key Responsibilities:
  • Conduct spiritual assessments as part of the initial patient evaluation.
  • Provide non-denominational or faith-specific spiritual counseling to patients and their families.
  • Offer crisis support and guidance through grief, fear, or existential distress.
  • Coordinate and lead spiritual rituals, services, or prayer if requested.
  • Respect and honor diverse religious backgrounds and individual belief systems.
  • Collaborate with the interdisciplinary team during care plan development and IDG meetings.
  • Document all chaplain visits and services in the electronic medical record (EMR) per regulatory standards.
  • Provide spiritual support to staff and participate in debriefings or memorial services as needed.
  • Make appropriate referrals to community clergy or faith-based organizations when requested.
Qualifications:
  • Education: Master of Divinity, Theology, or related field from an accredited seminary or theological institution.
  • Certification: Board Certified Chaplain (BCC) through APC, NACC, or equivalent strongly preferred; or working toward certification.
  • Experience: At least 1 year of experience in hospice, palliative care, hospital ministry, or pastoral care.
  • Endorsement: Must have ecclesiastical endorsement from a recognized religious body.
  • Licensure (if applicable): Compliance with state requirements for clergy or spiritual care providers.
  • Skills: Compassionate, culturally sensitive, non-judgmental, and an excellent listener.
  • Transportation: Valid driver’s license and reliable transportation for home visits.
Working Conditions:
  • Flexible, part-time schedule (e.g., 10–20 hours per week), including occasional evenings or weekends.
  • Travel to patient homes, nursing facilities, or inpatient units as needed.
  • Emotional resilience required due to the sensitive nature of end-of-life work.
