HomeWell Care Services supporting Cherokee County is seeking a compassionate and confident Registered Nurse Care Manager. This part-time role is ideal for a newly licensed RN interested in care management, senior care, and building trusted relationships with clients and families. Training and mentorship will be provided.
This is a hybrid position based in Canton, Georgia. Some administrative work may be completed remotely, but local travel is required for client assessments, supervisory visits, and caregiver support.
Responsibilities
  • Conduct in-home client assessments and reassessments.
  • Develop and update individualized care plans.
  • Explain HomeWell’s services and care recommendations to prospective clients and families.
  • Build trust, answer questions, and help prospective clients make informed care decisions.
  • Complete required supervisory visits.
  • Monitor service quality and changes in clients’ needs or condition.
  • Communicate with clients, families, caregivers, healthcare professionals, and office staff.
  • Guide caregivers in following client care plans.
  • Maintain accurate and confidential documentation.
  • Assist with care coordination, quality assurance, caregiver education, and incident follow-up.
Qualifications
  • Active, unrestricted Georgia Registered Nurse license.
  • Newly licensed RNs are encouraged to apply.
  • Strong communication, organization, and time-management skills.
  • Confidence discussing care needs and HomeWell’s services with prospective clients and families.
  • Compassionate and professional approach to working with older adults.
  • Ability to work independently and collaboratively.
  • Valid driver’s license, reliable transportation, and current automobile insurance.
  • Ability to travel throughout Canton and Cherokee County.
  • Basic proficiency with email and electronic documentation systems.

Schedule and Compensation
