Job Summary and Responsibilities

As our Clinical Pharmacist, you will help our doctors and nurses safely prescribe and dispense medications to help our patients feel better and recover from their illness or injury.
Every day you will build trust in these interactions by reviewing every prescription in your queue. We never rely entirely on the computer to check for safety; we always use an expert for the final check. That’s you. You’ll review prescriptions for potential drug interactions, correct dosing, patient allergies or anything that looks unusual in your opinion.
To be successful in this role, you will enjoy learning new concepts and researching information in real-time to provide solutions for our patients. You should also continually look for ways to go above and beyond for your patients, delivering an exceptional experience along with exceptional care.

  • Performs at least two medication utilization and disease state reviews each year to assess prescriber compliance with medical staff approved guidelines. Works with the Infection Control and Pharmacy and Therapeutics Committees to improve adherence to guidelines.
  • Adjust medications for renal insufficiency in accordance with medical staff approved renal dosing protocol as indicated.
  • Converts therapy from IV to PO in accordance with medical staff approved IV to PO conversion policy as indicated.
  • Documents medication errors and adverse drug reactions.
  • Documents clinical interventions. Tracks and documents cost savings related to clinical interventions, prevented adverse drug reactions, medications errors and secondary infections.
  • Monitors facility specific and Dignity Health-EV antibiogram on a quarterly basis and suggests modifications to formulary/use as needed.
Job Requirements

Required

  • Bachelors of Science in Pharmacy or Doctor of Pharmacy 
  • Active Pharmacist license in the State of California

Preferred

  • Five (5) years years hospital experience and clinical pharmacy residency
  • Experience in developing an antimicrobial stewardship program or participating in an established antimicrobial stewardship and implementing medication safety program.
  • Experience presenting at the Pharmacy and Therapeutics Committee 
  • Policy writing for the management of new medications and submission to the physician's committee for review and approval 
  • Ability to staff pharmacy and clinical areas such as ICU, etc.

Required
