JOB SUMMARYย 

The Nurse Practitioner role involves providing advanced healthcare services, diagnosing illnesses, prescribing medications, and collaborating with interdisciplinary teams to deliver holistic patient-centered care. With expertise in primary and specialty care, Nurse Practitioners play a vital role in improving health outcomes and enhancing the quality of life for patients.

We are seeking aย full-time Advanced Practice Provider (Nurse Practitioner or Physician Assistant)ย to deliver high-qualityย Primary Careย within aย Value-Based Care (VBC)ย model. This role is ideal for an APP experienced in managing chronic conditions, improving outcomes, and supporting population health initiatives through coordinated, proactive care.

DUTIES & RESPONSIBILITIESย 

  • Maintains confidentiality of all patient records.ย 
  • Checks in with clients about test results and prognoses.ย 
  • Advises patients regarding prescriptions and treatment plans.ย 
  • Follows all federal, state, and nursing guidelines, regulations, and standards.ย 
  • Orders and interprets diagnostic tests, including x-rays, blood work, and scans.ย 
  • Refers patients to specialists if necessary and analyzes medical records and reports.ย 
  • Updates patient charts and uploads to Electronic Health Record (EHR) software.ย 
  • Works with a team of nurse practitioners, registered nurses, and doctors to provide quality patient care.ย 
  • Examines patients, including discussing medical history and symptoms and performing physicals.ย 
  • Knows how to use popular tools, including: medical database software, medical research software, medical diagnosisย software, medical billing software, e-prescribing software, scheduling software, billing software, and telemedicine software.ย 

EDUCATION AND PROFESSIONAL EXPERIENCEย 

  • Nurse Practitioner and Registered Nurse licenses in the state of Florida.ย 
  • Board certification as a Nurse Practitioner in the specialty is preferred.ย 
  • National board certification as a Nurse Practitioner from an accredited agency.ย 
  • Master's in nursing required, Doctorate of Nursing preferred.ย 
  • Two or more years of clinical experience as a nursing practitioner at a physician practice or in a hospital. Nurse Practitioner candidates must have prior Value-Based Care experience, including experience with quality measures, risk-based/panel management, care coordination, outcomes-based care delivery, and documentation practices that support accurate clinical capture and continuity of care.
  • Preferred Clinical Experience (Renal/Nephrology) - Renal and/or nephrology experience isย highly preferred, including familiarity with CKD management, hypertension, diabetes-related renal disease, electrolyte issues, and coordination of care for patients with renal impairment (including dialysis coordination when applicable).
  • Bilingual in English & Spanish preferred.

PROFESSIONAL COMPETENCIESย 

  • Strong foundation in Primary Careย (Internal Medicine and/or Family Medicine preferred)
  • Comfort managing medically complex, chronically ill patient populations.
  • Communicates effectively, speaking multiple languages is preferred.
  • Collaborates with nurse practitioners, registered nurses, doctors, and other medical professionals.ย 
  • Problem-solves well independently and as part of a team.ย 
  • Develops strong relationships with patients and their families and has a stellar bedside manner.ย ย
