Primary Care Physician (MD/DO)

Value-Based Medicare Advantage | Relationship-Driven Care

The Opportunity

Join a physician-led primary care organization built around outcomesโ€”not volume. This role offers manageable patient panels, strong care team support, and the ability to practice medicine the way it was intended: with time, accountability, and purpose.

Your Role
  • Provide comprehensive primary care to older adult patients

  • Maintain full accountability for clinical outcomes and patient experience

  • Serve as a health coach, guiding lifestyle and long-term health improvement

  • Coordinate specialty and hospital care while staying actively involved

  • Lead and collaborate with an interdisciplinary care team

  • Supervise and remain accountable for Advanced Practice Providers (APPs)

  • Partner with medical and operational leadership to meet quality and cost goals

  • Appointment utilization (18โ€“21 slots per day)

Qualifications
  • MD or DO required

  • Board Eligible or Board Certified in Internal Medicine, Family Medicine, or Geriatrics

  • Active or eligible state medical license
