Job description

Job Summary
Our client is seeking a dedicated Registered Nurse to oversee the care of participants within the PACE program. The primary responsibilities include conducting clinical assessments, developing care plans, and providing direct patient care. The nurse will also support outpatient clinic operations, collaborating with healthcare professionals to enhance patient outcomes.
Key Responsibilities
  • Oversee 70-75 participants within the PACE program.
  • Conduct initial, semi-annual, and ongoing nursing assessments.
  • Develop and update individualized nursing care plans.
  • Provide skilled nursing care, administer medications, and monitor health status.
  • Ensure medication adherence and provide patient education.

Qualifications
  • RN (WA certified)
  • 2 years of experience in outpatient settings.

Skills
  • Experience with outpatient care.
  • Preferred EPIC and geriatrics experience.

Schedule/Shift
M-F 8-5
