RN Clinical Manager โ€“ Home Health

About the Role

The RN Clinical Manager is responsible for leading clinical operations within a home health setting, ensuring high-quality, compliant, and patient-centered care. This role oversees clinical staff, coordinates care delivery, and drives performance-improvement initiatives to enhance patient outcomes and operational efficiency.

Youโ€™ll serve as a key leader, supporting interdisciplinary collaboration while ensuring adherence to regulatory standards and agency policies.


Key Responsibilities
  • Lead and oversee clinical operations to ensure safe, high-quality patient care in compliance with state and federal Conditions of Participation (CoPs)
  • Supervise and support clinical staff, ensuring appropriate care planning, reassessment, and continuity of services
  • Coordinate and collaborate with interdisciplinary teams to optimize patient outcomes
  • Monitor and manage the QAPI program, implementing corrective actions as needed
  • Ensure compliance with Medicare, Medicaid, and other third-party payer requirements
  • Identify operational or clinical issues and communicate findings to leadership with actionable recommendations
  • Promote infection control practices and adherence to safety standards
  • Support staff development, training, and ongoing education
  • Participate in interdisciplinary group (IDG) meetings and performance improvement initiatives
  • Foster a culture of accountability, communication, and clinical excellence

Qualifications
  • Current Registered Nurse (RN) license (required)
  • Minimum 2+ years of home health management experience
  • Strong knowledge of home health regulations and compliance standards
  • Experience with HomeCare HomeBase (HCHB) preferred
  • Current CPR certification
  • Valid driverโ€™s license and auto insurance
