Position Overview
The Triage Registered Nurse is an experienced nurse responding to overnight and weekend calls regarding the changing needs of hospice patients. The Triage Registered Nurse is responsible for answering questions, gathering information, assessing patient needs and when needed, coordinating and directing hospice services/care via phone conference. The Triage Registered Nurse is experienced in nursing with an emphasis on palliative care.
To be considered, candidates must reside in our service areas: MN, WI, IA, IL, NE, SD, MO, KS, MI, IN
Full-time Schedule is only 3days/week: Friday 4pm-9pm, Saturday and Sunday from 6am - 8pm/ off Monday - Friday
Essential Functions and Skills
Patient Care

  • Performs all duties and responsibilities in accordance with the Nurse Practice Act and in accordance with basic principles and guidelines of professional nursing.
  • Receives calls related to patient care and instructs patient/caregiver on appropriate interventions based on nursing knowledge.
  • Evaluates available options for interventions and uses critical thinking when selecting which intervention is best for the patient and/or caregiver.
  • Determines the need for nursing visit based on EMR and telephone call. Dispatches a visit RN or LPN as appropriate.
  • Completes, submits, and maintains accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management and changes/outcomes as appropriate.
  • Communicates with the physician regarding patient needs and reports changes in patient condition; obtains/receives physician orders as required.
  • Facilitates the patient's efforts toward self-sufficiency and optional comfort care.
  • Counsels and works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver.
  • Ensure arrangements for equipment and other necessary items and services are available.
  • Responsible for maintaining compliance with all HIPAA regulations and requirements. Treats all patient information as confidential and utilizes protected health information on a minimum necessary basis.

Additional Duties
  • Assumes responsibility for personal growth and development. Maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in- service classes.
  • Actively participates in quality assessment and performance improvement teams and activities as assigned.
  • Other duties as assigned.

Requirements/Qualifications
  • Must have a current Registered Nurse (RN) license in state(s) of practice.
  • Nurses holding a multistate license can practice in other states without obtaining additional licenses while maintaining their Registered Nurse (RN) license in their primary state of residence (PSOR). Minnesota, Michigan, and Illinois are not in the Nursing Licensure Compact.
  • Possess or obtain an RN license for the 10 states that we serve and/or a compact license to cover the states before hired. States include MN, WI, IA, SD, IL, MI, MO, IN, KS and NE.
  • Graduate of a professional school of nursing and is currently licensed, certified or registered in the state(s) of practice.
  • Ability to pass DHS background, consumer report, and drug test.
  • If Driving: Valid State Driver's License, automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record required.

Preferred
  • 1+ years of recent experience in hospice/home care preferred.
  • 2+ years of Nursing experience, preferred

Physical Requirements
The physical requirements described are representative of those that must be met to successfully perform the essential responsibilities of this position. Reasonable accommodation may be made.
  • Pushing/Pulling and Lifting/carrying up to 30 pounds. Occasionally up to 50 pounds.
  • Ability to adapt to a variety of work conditions to ensure the highest quality of patient care in a home setting.

The salary range for this role is $75,000 - $85,000/salary. An employee's actual annual salary will be based on but not limited to: geographic location, relevant education, applicable certifications, depth of experience, and special skillsets. The total compensation package for this position may also include the opportunity to participate in employee benefits including our medical/dental/vision/pet insurance, disability and life insurance, paid time off, and 401(k) retirement plans, subject to applicable plan terms, and/or eligibility for other compensation.
