Office Location: 1811 Weir Dr., Ste. 150, Woodbury, MN 55125 
Coverage area:Woodbury / Forest Lake 

Schedule: (Overnight)Mon-Thur, 5:00 PM - 8:00 AM 

Our On-Call RNs play a crucial role in end-of-life care, stepping in during off-hours to bring relief, clarity, and compassion when it's needed most. HOSPICE OF THE MIDWEST is seeking a passionate, dedicated RN to join our team inWoodbury, MN. As a On-Call RN You will: 

  • (CHOOSE ONE: If office HAS a Remote Triage team) Collaborate with Remote Triage On Call Team to deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis  
  • (CHOOSE ONE: If office DOES NOT have Remote Triage) Deliver hands-on care to hospice patients and offer supportive services to their families during after-hours, on an on-call basis 
  • Assess and manage urgent symptoms quickly and compassionately 
  • Administer medication or adjust care based on hospice protocols or provider orders 
  • Communicate with the physician regarding changes in condition and obtain orders as necessary 
  • Offer guidance and support to the patient and family in understanding the current condition and anticipating the next steps 
  • If a death occurs during on-call hours, the RN will go to the home or facility to pronounce death, provide support to the family, contact the funeral home, and notify the hospice team
