The Home Health RN will report to a Clinical Manager and collaborate closely with friendly staff and a diverse team of physicians, advanced practice providers, pharmacists, PT/OT/SLP, social work, case management, and scheduling/triage support. As a Home Health RN, responsibilities include comprehensive assessments and OASIS documentation, individualized care plans, wound and ostomy care, IV infusions, line and catheter management, patient and caregiver education, safe transitions of care, payer communication, and continuous quality improvement supported by clinical decision tools, optimized EHR builds, and real-time analytics dashboards.
