Join AmeriCorps as a Health Educator with a focus in Health Promotions at Erie Family Health Centers. This full-time service year opportunity supports health education, chronic disease prevention, patient engagement, and community outreach for underserved populations. Members provide one-on-one health education, facilitate group sessions, conduct patient outreach and disease management assessments, and help patients develop healthy lifestyle habits through in-person and telehealth services. This role is ideal for individuals who are passionate about compassionate patient care, strong communication, and collaborating with healthcare teams to improve health outcomes in the community.

This is a year-long, full-time AmeriCorps position requiring completion of a minimum of 1,700 service hours. The work schedule for this position is aligned with Erie's hours of operations.
Main Duties & Responsibilities
  • Provide individualized health education and lifestyle coaching to patients, helping them manage chronic conditions and improve overall health outcomes through in-person and telehealth visits.
  • Assess patient needs, identify barriers to care, and connect patients with appropriate programs, resources, and support services.
  • Facilitate health education groups and disease prevention programs while recruiting and engaging patients to participate in available services.
  • Conduct patient outreach, follow-up, appointment scheduling, and care gap re-engagement activities to support continuity of care.
  • Collaborate with healthcare providers, care teams, and community partners to coordinate services, support program goals, and improve patient outcomes.
  • Participate in community engagement activities, health fairs, educational events, and quality improvement initiatives that promote health and wellness in underserved communities.

Education
Required:
  • High school diploma or equivalent.

Skills and Knowledge
Required:
  • Strong verbal and written communication skills.
  • Ability to work with diverse populations.
  • Current CPR certification
  • U.S. citizen, national, or lawful permanent resident.
  • Ability to meet AmeriCorps background check requirements.

Preferred:
  • Experience in community health, social services, or related fields.
  • Intermediate Microsoft Office skills.
  • Experience with electronic health record systems.
  • Bilingual English/Spanish skills.
