PROLOGUE JOB DESCRIPTION

Position Title: Adult Outreach Worker, Homeless Outreach Program

General Description:

The Adult Outreach Worker identifies and engages people living in unsheltered locations, such as in cars, parks, encampments, abandoned buildings and on the streets in the Baltimore County. The Outreach Worker works with people who might not otherwise seek assistance and ensures that their immediate basic needs are met at the point of engagement. The Outreach Worker is responsible for conducting outreach, providing information and referrals, and rendering person-centered services. The Outreach Worker will also provide services at our Homeless Outreach Center.

Schedule: Full-time, Monday through Friday

FLSA: Non-exempt

Reports to: Program Director

Required Education, Knowledge and Skill Abilities to Perform Essential Job Functions:

An Associate's degree is required, but a Bachelor's degree with a major in Human Services, Psychology, Social Work, Sociology, or Education is preferred. Experience working with special populations is also preferred. Flexibility and the ability to work as part of a team are necessary. A valid Maryland Driver's License with a clean driving record (less than one point) is also essential. Proficiency with Microsoft Office, the internet and other computer applications is required. Ability to respect individual choice and a commitment to serving the homeless population is a must. This position requires excellent verbal and written communication skills, organizational skills, and the ability to handle multiple tasks at once. The ideal candidate is compassionate, adaptable, flexible, creative, takes initiative and able to work independently.

Physical Requirements: This position requires daily driving, standing, lifting up to 25 pounds, and working outside in all types of weather year-round.

Summary of Essential Functions, Responsibilities and Duties:

• Provide direct, intensive and consistent outreach services to Baltimore County residents experiencing homelessness in accordance with grant requirements and program outcomes. Services rendered will be provided in various settings, including encampments, on the streets and in other community areas.

• Maintain a person-centered approach in the deliverance of services.

• Link, advocate and support clients in obtaining requested services and resources.

• Complete detailed documentation of services rendered on a monthly, quarterly and annual basis as required.

• Submit all reports in a timely manner and enter data into HMIS as appropriate.

• Collaborate with community providers to establish professional networks, referral sources and provide education on program services.

• Attend and participate in provider and agency meetings as scheduled.

• Perform Outreach Center coverage on a bi-weekly basis.

• Ensure compliance with all Prologue and Homeless Outreach Program policies and procedures as prescribed.

• Ensure fiscal compliance with agency equipment, vehicles and funds.

• Utilize computer systems for external/internal communication, Relias online training and Time/Attendance on consistent weekly basis to meet payroll deadlines.

• Perform other duties as assigned by supervisor.
