Community Care Network is looking for a Community Outreach Specialist to join our team!
Community Care Network is comprised of Rutland Mental Health Services and Rutland Community Programs. Our mission is to enhance the well-being of our communities, individuals, and families through responsive, innovative, and collaborative human service
About the Role:
The Community Outreach Specialist provides support services to consumers residing in an independent housing program and in the community. The support services include coordinating client case management, planning and carrying out goals of individual's treatment plan/ISA, goal setting, training independent living skills, and supporting individuals' overall mental health stability and general wellbeing. The Community Outreach Specialist will provide transportation of day-to-day appointments as needed. This support service assists individuals to reside independently in the community through skill development and education. This position allows for interfacing with a variety of individuals with differing needs and approaches.
Principal Responsibilities:
  • Serve as a role model, mentor, advocate, and motivator to recovering individuals in order to help prevent relapse and promote long-term recovery and mental health stability.
  • Assist in planning and implementing clients' individual plans of care by coordinating Community Case Management services with Clinician/Case Managers/Service Coordinator as appropriate.
  • Respect individuals' choices and encourages advocacy.
  • Provide a professional and individualized support system that helps to supervise, direct, inform, and assess current and prospective needs of individual.
  • Demonstrate knowledge of individual characteristics such as physical health, mental health, psychosocial characteristics in order to address needs in a personalized manner.

Qualifications:
  • High school Diploma or GED required; Bachelor's Degree in Psychology, Counseling, Social Work or similar field preferred.
  • Ability to work alternative work schedules, evenings, weekends, and holidays as needed for operations and at the request of management.
  • Flexibility, dependability, and ability to work independently.
  • Must have a valid driver's license, reliable transportation with personal automobile liability insurance coverage at or above the levels mandated by the State of Vermont.
  • *Successful completion of a criminal history background check after hire.

Requirements:
  • Must be able to work Sunday through Wednesday from 11AM - 9PM
