Job description

Our Lady of Consolation is committed to improving our patients' quality of life & autonomy through innovative rehabilitative services, and an exceptional level of client-centered care and attention. We are seeking full time Speech Therapist to join our interdisciplinary team of health care professionals at our skilled nursing facility in West Islip.

Responsibilities include but are not limited to:

  • Conduct speech and swallow evaluations to diagnose & develop care plans.
  • Refer clients to additional medical or educational services if needed.
  • Monitor residents' progress and make modifications to care plans as needed.
  • Record information on the initial evaluation, treatment, progress, and discharge of clients.
  • Observe and assess the progress & prepare reports and maintain all necessary documentation.
  • Ensure compliance with laws, regulations, and standards.

Requirements:

  • A graduate of an accredited SLP Program.
  • Current New York State SLP License.
  • Experience in long-term care settings preferred.
  • Strong documentation skills.
  • Comfortable working with sick, older adults.
  • Experience working in a LTC setting preferred.
