Vertis Therapy is seeking an experienced Program Director Occupational Therapist (OT), Certified Occupational Therapy Assistant (COTA), or Physical Therapy Assistant to join our Assisted Living community in a full-time role. This position is ideal for a motivated clinician who enjoys one-on-one patient care, has a focus in behavioral health/psychology, and plays a key role in day-to-day clinical operations-without the stress of high-acuity settings.

The Program Director's primary responsibility is for leadership of the therapy team including management of the day-to-day operations of the therapy department, the supervision of therapy staff, and the coordination of RUG management. The secondary responsibility is patient care.  The Program Director exhibits high performance in Vertis core values, leadership, team development, customer service, and department operations.  The Rehab Director works closely with the Regional Manager to ensure that Vertis standards are maintained and that available resources are well utilized.

Primary Responsibilities

  • Ensures that rehabilitation services are provided in compliance with Vertis and facility policies and procedures; State, Federal, and professional organization regulatory standards and guidelines.
  • Coordinates resident screening (PT/OT/SLP) programs.
  • Serves as advocate for meeting the rehabilitation needs of residents.
  • Coordinates attendance at care plan and departmental meetings.
  • Acts as a liaison between therapy department, administrator, and nursing staff.
  • Ensures accurate and timely month-end billing.
  • Coordinates therapist schedules and arranges prn staffing as needed.
  • Communicates PT/OT/SLP schedules and changes to administration and nursing staff.
  • Coordinates the execution of the Restorative Program with nursing staff and therapy department.
  • Prepares the Rehab Department for a deficiency free State Survey and acts as department representative during the survey process.
  • Completes Vertis guide to survey twice annually.
  • Trains staff in Vertis and facility policies and procedures.
  • Assists in identifying therapy needs of resident population.
  • Leads therapy team in implementing clinical programs.
  • Effectively communicates pertinent information from Vertis management and facility management to therapy team.
  • Assists Regional Manager in Performance Reviews, applicant interview process, and disciplinary actions.
  • Assists Regional Manager in the orientation of new employees and contract therapists.
  • Supports team development by encouraging individual growth, recognizing accomplishments, and providing a positive work environment.
  • Performs other duties as assigned by Regional Manager
  • Analyzes team productivity levels and facilitates achievement of company expectations.Assists Regional Manager in analyzing key monthly operational data to achieve company success. 
  • Provides direct patient care, applying the principles, methods, materials and equipment used in own discipline to the patient population served in this facility.

Minimum Qualifications/Requirements



  • Associate of Science degree in Physical Therapy Assistant or Occupational Therapy Assistant.
  • Current license/certification in the state of Indiana.
  • Minimum one (1) year experience in rehabilitation setting serving the same age/type of patient population served by this facility.
  • Excellent communication and interpersonal skills.
