The Role

The Director of Nursing will provide clinical leadership and oversight for Village Home Health's nursing and patient care operations. This individual will be responsible for supporting clinical excellence, regulatory compliance, staff development, quality outcomes, and the day-to-day coordination of patient care.

We are looking for a leader who can balance clinical quality with operational efficiency, build strong relationships with staff and providers, and create a culture of accountability, communication, and compassionate care.

Key Responsibilities
  • Provide leadership and oversight of all clinical home health operations.
  • Supervise, mentor, and support nursing and clinical staff.
  • Ensure patient care is delivered safely, effectively, and in accordance with physician orders and established plans of care.
  • Maintain compliance with applicable federal and state regulations, CMS requirements, Medicare Conditions of Participation, and agency policies.
  • Oversee clinical documentation, care planning, OASIS processes, and timely completion of required records.
  • Monitor patient outcomes, hospitalizations, incidents, complaints, and other clinical quality indicators.
  • Lead and participate in quality assurance and performance improvement initiatives, including QAPI activities.
  • Review clinical cases to identify care gaps, risks, delays, and opportunities for intervention.
  • Partner with administrative leadership to improve patient experience, clinical outcomes, productivity, and operational performance.
  • Support recruitment, onboarding, orientation, competency validation, education, and ongoing development of clinical staff.
  • Promote timely and effective communication between patients, families, physicians, referral sources, caregivers, and members of the interdisciplinary team.
  • Ensure appropriate staffing and caseload management while maintaining high standards of care.
  • Participate in survey preparation and maintain ongoing survey readiness.
  • Develop and reinforce policies, procedures, and clinical standards that support consistent, high-quality care.
  • Foster a positive culture where employees feel supported, accountable, and empowered to provide excellent patient care.
Qualifications
  • Current, unrestricted Registered Nurse (RN) license in the applicable state.
  • Minimum of 3 years of nursing experience, with home health experience required.
  • Prior nursing leadership, supervisory, clinical management, or Director of Nursing experience preferred.
  • Strong understanding of home health clinical operations and regulatory requirements.
  • Knowledge of Medicare Conditions of Participation, OASIS, care planning, documentation standards, and quality improvement processes preferred.
  • Demonstrated ability to lead, coach, and develop clinical teams.
  • Strong organizational, problem-solving, and decision-making skills.
  • Excellent written and verbal communication skills.
  • Ability to manage multiple priorities while maintaining attention to clinical quality and patient safety.
  • Comfortable using electronic medical record systems and other healthcare technology.
