This role is primarily remote, allowing flexibility in managing your work schedule while delivering essential services to our clients at the detox center. The typical work hours are from Monday to Friday, 8:00 AM to 4:30 PM, though some evening or weekend availability may be required based on client needs and emergencies. The remote nature of the work fosters a modern approach to healthcare service delivery, providing you with the tools and resources to effectively engage with clients from your home while ensuring all necessary support is readily available.

• Conduct comprehensive assessments of seniors' psychosocial needs, focusing on emotional, mental, and social factors that influence their well-being.
• Develop personalized care plans in collaboration with clients and their families, ensuring their specific goals and values are included.
• Provide empathetic counseling and emotional support to seniors facing life transitions, such as recovery from addiction or adapting to new living arrangements.
• Actively advocate for clients’ rights and access to critical community resources, ensuring they receive the necessary support services.
• Work closely with a multidisciplinary team, including medical professionals, caregivers, and mental health specialists, to coordinate comprehensive care for our clients.
• Regularly visit clients remotely to monitor their well-being, evaluate living conditions, and address safety concerns, utilizing virtual communication tools effectively.
• Facilitate healthy communication among family members, aiding in the resolution of conflicts and improving support dynamics.
• Educate clients and their families about available community resources, support groups, and programs to foster social engagement.
• Maintain meticulous records of client interactions, assessments, and care plans in compliance with ethical and organizational standards.
• Continuously update your knowledge of social work practices and interventions, ensuring adherence to current regulations and best practices.

This position primarily focuses on seniors dealing with issues related to detoxification and recovery. You will support clients through their healing journey, addressing mental health concerns while considering their unique social and emotional contexts. • A Master’s degree in Social Work from an accredited institution is essential.
• Current licensure as a Licensed Clinical Social Worker (LCSW) is required.
• A minimum of two years of experience in social work, preferably in a community, geriatric, or healthcare setting.
• Strong understanding of social work principles and aging-related issues, alongside familiarity with community resource networks.
• Exceptional assessment, counseling, and communication skills.

• Experience working within a detox or rehabilitation center.
