Description
The MDS Coordinator will be responsible for coordinating the Minimum Data Set (MDS) assessment process, ensuring accuracy and compliance with regulatory requirements. This position requires strong clinical assessment skills, proficiency in MDS software, and a commitment to maintaining the highest quality of care for our residents.
Schedule: Full-Time, Weekdays, On-Call, Some Weekends, Rotating On-Call
Primary Responsibilities and Duties:

  • Coordinate the completion of the MDS assessment process for all residents according to federal and state regulations and facility polices.
  • Conduct comprehensive resident pre-assessment and/or assessment using standardized assessment tools, gathering information on residents' physical, psychological and psychosocial status.
  • Collaborate with team members including registered nurses, therapists, social workers, and dietary staff, to gather assessment data and develop individualized care plans.
  • Ensure accuracy and completeness of MDS assessments, reviewing documentation for completeness, consistency, and compliance with regulatory requirements.
  • Coordinate the scheduling of MDS assessments and ensure timely completion of assessments for new admission, quarterly reviews, significant change assessments, and discharge assessments.
  • Utilize MDS software to enter assessment data, generate reports, and maintain resident records in compliance with regulatory requirements.
  • Monitor changes in residents' condition and update MDS assessments as needed to reflect changes in care needs and ensure accuracy of reimbursement levels.
  • Participate in care conferences and team meetings to discuss residents' care plans, progress, and any changes in condition or care needs.
  • Collaborate with billing and reimbursement staff to ensure accuracy of Medicare, Medicaid, and insurance billing based on MDS assessment data.
  • Provide education and training to staff members on MDS assessment protocols, documentation requirements, and regulator changes.
  • Complete reports and assignments accurately and adhere to established time schedules.
  • Perform all other duties assigned.
Requirements
Job Requirements:
  • Valid license as a Registered Nurse or Licensed Practical Nurse (RN/LPN) in the state where work is performed.
  • Minimum of 1-3 years experience working as an RN in a healthcare setting. With experience in MDS coordination preferred.
  • Strong clinical assessment skills and knowledge of nursing principles, procedures, and best practices in long-term care.
  • Proficiency in MDS software and electronic health record systems.
  • Excellent organizational and time management skills, with the ability to prioritize tasks and meet deadlines in a fast-paced environment.
  • Understanding of federal and state regulations governing MDS assessments, reimbursement system, and skilled nursing facility operations.
  • Excellent communication and interpersonal skill, with the ability to collaborate effectively with team members, residents and their families.
  • Attention to detail and accuracy in documentation with a commitment to maintain compliance with regulatory requirements.
  • Ability to build rapport with patients through a compassionate and caring demeanor.
Physical Requirements:
  • Work performed in a medical setting due to the need to work in person with residents.
  • Must be able to sit, stand, bend, reach, talk, hear, stoop, kneel, crouch, stretch, and twist.
  • Manual dexterity and hand-eye coordination for using computers, entering data into MDS software, and completing documentation.
  • Stamina to stand and move about the facility to perform frequent rounds and conduct resident assessments.
  • Emotional resilience and ability to cope with the demands of coordinating MDS assessments and ensuring compliance with regulator reequipments.
  • Visual acuity and auditory ability to assess residents' condition, read medical records, and communicate effectively with staff and residents.
  • Maintain regular attendance and timeliness as set forth within the work schedule designated by the department supervisor/manager.
