Job description


The North Lambton Community Health Centreis a community-governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario:Forest, Kettle & Stony Point, Watford and Sarnia. Our interdisciplinary team of providers includes physicians, nurse practitioners, registered nurses, social workers, health promoters, dietitians and physiotherapists.

The North Lambton Community Health Centre is currently seeking applicants for the following position:

Registered Nurse or Registered Practical Nurse

37.5 hours per week - One Year Contract

The Outreach Nurse - Hepatitis C & Substance Use Health is a member of the interdisciplinary Hepatitis C & Substance Use Health Outreach Team at the North Lambton Community Health Centre. The position may be filled by a Registered Nurse (RN) or Registered Practical Nurse (RPN), with duties assigned in accordance with the successful candidate's scope of practice, individual knowledge, skill and judgment, organizational policies, and client care needs.


Qualifications:

  • Current registration in good standing as an RN or RPN with the College of Nurses of Ontario.
  • Minimum three (3) years of recent nursing experience.
  • Demonstrated ability to work independently, manage competing priorities, make sound nursing decisions, and recognize when consultation or escalation is required.
  • Demonstrated experience completing nursing assessments, obtaining comprehensive health and medication histories, developing care plans, and providing follow-up for clients with complex health and social needs.
  • Current competence in venipuncture and blood specimen collection or demonstrated ability to obtain competency during orientation.
  • Demonstrated experience working with individuals experiencing complex health and social needs.
  • Knowledge of:
    • Harm reduction principles
    • Social determinants of health
    • HIV and Hepatitis C transmission, prevention, and treatment
    • Substance use and its impacts on health
  • Experience in case management and community system navigation.
  • Experience providing services to marginalized populations, including Indigenous communities.
  • Strong computer skills, including the use of MS Office products and Electronic Medical Record.

This is a contract position, working 37.5 hours per week, Monday to Friday. Hourly wage for a Registered Nurse in this position is ($36.62-$43.49) and hourly wage for a Registered Practical Nurse in this position is ($26.68-$31.69). Experience will be taken into consideration for each 6-step wage grid. The incumbent will be required to work at all our sites and outreach locations as necessary.
