Registered Nurse - Hospice 7/7

The Hospice RN is responsible for the delivery of professional nursing services to hospice patients as indicated by the plan of care developed by the interdisciplinary team. Responsibilities include initial and on-going assessment of patients, developing and revising the plan of care, timely documentation, and communication of assessments, planning and interventions, coordination of care among team members.
Note: This is a 0.8 position with a 7 on / 7 off schedule working night shifts.

Estimated Hiring Range:

$113,940.00 - $139,260.00

Essential Responsibilities

* Performs initial and ongoing nursing evaluations of patients/family/caregiver and assists in the development/implementation of the plan of care. Nurses may be responsible for one or more of the following areas:

o Case management

o Admissions

o Initial hospice assessments

o Routine or focused nursing visits

o Telephonic triage

o Night/weekend coverage

o Coverage for increased census or staffing shortages

o Other duties assigned by the Hospice Leadership

* Responsible to coordinate and monitor all care being provided and keeps the physician and other team members from agency and community resources apprised of changes in patient's condition so that adjustments can be made to the plan or care as needed, to improve quality of life.

* Uses clinical assessment skills and best practices to identify and provide effective care for patients, families, and caregivers.

* Actively participates in IDT meetings for development and evaluation of the patient's plan of care

* Develop and implement a plan of care for each assigned patient/family/caregiver unit and evaluation of that plan of care.

* May assist patient/family/caregiver in determining of goals of care including completion of Advance Directives and POLST forms as needed.

* Provides dietary counseling as needed.

* Address advocacy needs for patients/families/and organization.

* Facilitates formal and informal meetings with patients/caregivers/family member as needed.

* Collaborates with the IDT during crisis management support and consultations with patient/family/caregiver or IDT team.

* Provides information regarding the patient/family/caregivers psychosocial, spiritual, physical, and financial needs or difficulties to the IDT team or providers involved in the patient's care.

* Serves as liaison with hospitals, care facilities and appropriate agencies to promote interagency cooperation, developing and maintaining communication and facilitating referrals to meet patient needs throughout the continuum of care.

* May participate in admission and on-call duties as assigned during the evenings and weekends.

* Participates in continuing education of self and assists with orientation of other team members.

* Collaborates as needed with Housecall Providers program staff to enhance continuity of care.

* Responsible for documentation in the electronic medical records, assuring accuracy, completeness and compliance with regulations, certification standards, legal and ethical standards per policies and procedures.

* Perform duties in compliance with Medicare Condition of Participation.

* Maintain patient/family and staff confidentiality in compliance with HIPAA laws.

* Participate in the coordination of the patient's hospice care in accordance with professional standards of practice.

* Practice within the scope of license, certification, or registration, where applicable, and as required by law and regulation.

* Develop and maintain professional boundaries with all patients/family/caregivers.

* Maintain agreed upon work schedule and meet deadlines for completion of work.

* Demonstrate the ability to function in a variety of care environments, including home settings, skilled nursing facilities, assisted living environment, etc.

* Serve as an ambassador for CareOregon and Housecall Providers at all times.

* Demonstrates cooperation and teamwork and maintains collaborative relationships with all levels of staff and community partners.

* Participate in orientation of new employees, volunteers or students seeking experience in hospice.

* Attend staff and mandatory in-services meetings, as required.

* Participate in ongoing education and assume responsibility for updating professional skills

* Familiar and comfortable with a person-centered approach to communication, education, and care planning.

* Maintains patient and personal safety regarding occupational health and safety and participates in safety program, as requested.

* Participates in Quality Assessment Performance Improvement efforts to continually assess and improve quality of care, as requested.

Experience and/or Education

Required

o Current unencumbered license as a registered nurse in the State of Oregon

o Minimum of two years of Registered Nurse medical/surgical experience in an acute care setting, and/or a combination of medical/surgical, home health, hospice experience or long-term care experience.

o 1 year of experience in quality process improvement/QAPI may substitute for 1 year of Registered Nurse experience

o CPR certification at hire or within 6 months in position

o Valid driver's license, acceptable driving record, and automobile liability coverage or access to an insured vehicle

Preferred

* Bachelor of Nursing degree or higher

* Certified in Hospice and Palliative Care (CHPN)

* 3-5 years in hospice triage, case management, admissions, night/weekend roles

Knowledge, Skills and Abilities Required

* Knowledge of Federal, State and Local regulations, Hospice Medicare Conditions of Participation, and HIPAA privacy rules and regulations

Travel: Requires travel outside of the workplace at least weekly; the employee's personal vehicle may be used. Driving infractions will be monitored in accordance with organizational policy.

Schedule: This position may occasionally require more than 8 hours per day.
