Position Overview

The Haven Family Therapy is currently seeking a Licensed Clinical Social Worker (LCSW) to join our dedicated team on a full-time basis at our hospice center located in Joliet, Illinois. This remote role offers the unique opportunity to provide vital emotional and social support to seniors and their families, contributing significantly to their quality of life during critical times.

Work Environment

This position allows for a flexible, remote work setting which facilitates a balanced work-life dynamic. While the standard work hours are Monday to Friday, from 8:00 AM to 4:30 PM, occasional evening or weekend hours may be required based on client needs. The role may also involve on-call duties to address urgent situations, requiring candidates to be adaptable and responsive to immediate client concerns. This flexibility ensures that our elderly clients receive continuous and uninterrupted care.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments, delving into emotional, mental, and social factors affecting seniors' well-being.
  • Work collaboratively with clients and their families to create and implement individualized care plans designed to meet specific goals and preferences.
  • Provide empathetic counseling and emotional support to seniors as they navigate complex life transitions, such as loss and end-of-life issues.
  • Act as an advocate for clients, ensuring they have access to essential community resources and services that enhance their quality of life.
  • Collaborate with interdisciplinary teams, including nurses and caregivers, to coordinate care and address the holistic needs of each client.
  • Conduct regular home visits to evaluate clients' living conditions, monitor well-being, and address any emerging concerns.
  • Facilitate communication among family members to resolve conflicts and enhance the care experience of the senior client.
  • Educate clients and their families about local resources, support groups, and programs available to foster meaningful social engagement.
  • Maintain detailed and accurate documentation of all assessments, care plans, and client interactions in adherence to ethical standards and company policies.
Patient Population / Client Focus

This role primarily focuses on elderly clients within a hospice setting. Social Workers in this role will engage with seniors experiencing chronic illnesses or nearing end-of-life, providing them with the necessary support through tailored interventions that respect their dignity and choices. Your expertise will directly enhance their comfort and peace of mind during challenging times.

Qualifications
  • A Master’s degree in Social Work (MSW) from an accredited institution.
  • Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • Minimum of two years’ experience in a social work role, ideally within a hospice or geriatric environment.
  • Strong understanding of social work ethics, principles, aging-related issues, and community resources.
  • Proficient assessment, counseling, and problem-solving skills, with an ability to establish rapport and trust with seniors and their families.
  • Excellent communication and interpersonal skills, enabling effective collaboration among interdisciplinary teams.
  • Ability to work efficiently with electronic health record (EHR) systems and relevant computer applications.
Preferred Experience
  • Previous experience in hospice care or within community health settings, specifically focusing on geriatric populations.
