The Opportunity

This home health RN Case Manager role is a high-impact clinical position where you'll own the patient journey from admission through discharge. You'll conduct OASIS assessments, manage complex care plans, deliver skilled nursing services, and lead an interdisciplinary care team — all while keeping patients safe, healthy, and out of the hospital.

Your Day-to-Day

  • Conduct comprehensive nursing assessments of patients' physical, emotional, psychosocial, and spiritual needs at admission, recertification, resumption of care, and discharge
  • Develop, implement, revise, and evaluate individualized plans of care in collaboration with patients, families, physicians, and the interdisciplinary team
  • Deliver skilled nursing care including wound care, medication management, IV therapy, injections, symptom management, and patient/caregiver education
  • Perform accurate, timely OASIS assessments to support proper reimbursement, quality reporting, and optimal outcomes
  • Educate patients and caregivers on disease management, medication adherence, safety, and self-care strategies to prevent re-hospitalization
  • Supervise and direct LPNs/LVNs, home health aides, and support staff to ensure coordinated, high-quality care
  • Monitor patient progress, identify changes in condition, and adjust care plans to optimize outcomes
  • Maintain accurate, timely clinical documentation in the EMR per agency policies, OASIS guidelines, and regulatory requirements
  • Participate in case conferences, IDT meetings, and quality improvement initiatives
  • Manage a productive caseload with efficient visit scheduling and ensure continuity of care across the service area
  • Respond to urgent patient needs and participate in on-call rotations as required

What We're Looking For

  • Graduate of an approved nursing school with current RN licensure in California
  • Minimum one year of registered nurse experience
  • Valid CPR certification
  • Valid driver's license, insured vehicle, and ability to travel to all patient locations
