Responsibilities:
  • Oncall hospice visits on weekends
  • Provide accurate and timely documentation
  • Assess and provide patient and family/caregiver education
  • Perform other duties as assigned by the registered nurse
  • Other visit as needed based on your availability including routine and PRN, visits, discharges, Deaths, CC, etc.
  • Qualifications:
  • Demonstrated compassion and commitment to patient centered care
  • Active LVN license in the state of California
  • Valid California driverโ€™s license and reliable transportation
  • Minimum 1 year of hospice experience; strong understanding of hospice philosophy and end of life care
  • Prior experience in nursing or related medical fields
  • Experience as an on call LVN runner preferred
