MDS Nurse (RN) / MDS Coordinator

Salary: $80,000-$85,000/year

Simpsonville Post Acute is seeking an experienced MDS Nurse (RN) to join our team in Simpsonville, SC. This position is responsible for coordinating and overseeing the resident assessment process, ensuring accurate and timely completion of MDS assessments, supporting PDPM optimization, and collaborating with the interdisciplinary team to achieve positive resident outcomes.

Job Responsibilities
  • Ensure timely and accurate completion of all MDS assessments, including Medicare PPS, Quarterly, Annual, Significant Change, and OBRA assessments
  • Coordinate the interdisciplinary MDS process and maintain compliance with federal and state regulations
  • Lead and participate in weekly PDPM meetings for skilled residents
  • Monitor PDPM performance and utilization of reimbursement tools
  • Analyze Quality Measures (QM) and QAPI/QASP data and provide recommendations for performance improvement
  • Assess new admissions and communicate resident level of care to interdisciplinary team members
  • Maintain assessment schedules and regulatory compliance timelines
  • Submit MDS assessments electronically and complete corrections as needed
  • Schedule and conduct resident care conferences
  • Collaborate with clinical staff to develop, review, and revise individualized care plans
  • Evaluate resident outcomes and ensure care plans are updated appropriately
  • Manage and communicate all facility level-of-care changes
  • Provide guidance and training to MDS staff and new MDS Coordinators
  • Maintain resident confidentiality and HIPAA compliance
Qualifications Required
  • Current, unrestricted Registered Nurse (RN) license
  • Graduate of an accredited School of Nursing
  • Knowledge of MDS assessment process and regulatory requirements
  • Strong organizational, communication, and documentation skills
  • Proficiency with Microsoft Office
Preferred
  • MDS Coordinator experience
  • Long-Term Care or Skilled Nursing experience
  • Experience with PDPM reimbursement processes
  • PointClickCare (PCC) experience
  • BSN preferred
  • Three or more years of supervisory experience
