Job description

Programs in Assertive Community Treatment (PACT) is an evidence-based outreach program to help people with serious mental illness integrate into the community. Bright Harbor Healthcare provides PACT services to more than 160 people in all of Ocean County. The PACT program is a multidisciplinary team for persons with a serious and persistent mental illness. A team consists of a psychiatrist, advanced practice nurse, nurses, clinicians, co-occurring specialists, vocational specialists, mental health specialists, and peer mental health specialists.

Position Title: Vocational Specialist II
Position Type: Full Time; 40 Hours/Weekly
Benefits Eligible: Full Benefits
Location: Bayville, NJ
Department: PACT II

Responsibilities:

  • Provide case management to assigned caseload
  • Participate in a multi-disciplinary treatment team for collaboration of consumer care
  • Provide crisis intervention as necessary
  • Engage all consumers in treatment planning and service delivery
  • Provide transportation as needed
  • Assist with ADL's through modeling

Requirements

  • Bachelor's Degree in Vocational Rehabilitation or related field
  • Two years of experience in vocational assessment, job preparation, or individualized job placement and/or job coaching with individuals with serious persistent mental illness (SPMI)
