Inpatient Care Manager (RN / Case Manager)

Location: Chicago, IL

Schedule: Full-Time | Monday-Friday | 8:00 AM - 4:30 PM

Position Overview

We are seeking an experienced and compassionate Inpatient Care Manager (RN/Case Manager) to support care coordination, discharge planning, and resource management across multiple inpatient units within a community-focused hospital setting.

This role is ideal for professionals who thrive in a collaborative environment and are passionate about improving outcomes for complex and underserved patient populations.

Key Responsibilities

  • Coordinate patient care and discharge planning across inpatient units

  • Collaborate with physicians, nurses, social workers, and interdisciplinary teams

  • Facilitate safe and timely patient transitions

  • Navigate insurance plans, authorizations, and post-acute placement needs

  • Identify and coordinate community-based resources for patients and families

  • Advocate for patients with complex social and medical needs

  • Maintain accurate documentation within EPIC EMR

Patient Population

  • Primarily adult and geriatric patients, including:

  • Unhoused and low-income populations

  • Patients with limited access to healthcare resources

  • Complex discharge and placement cases

  • Potential unit support may include:

  • Med/Surg

  • PCU

  • LDRP

  • Occasional ICU coverage

Caseload

  • Typically 14-20 patients

  • Required Qualifications

  • Minimum 3 years of acute care case management experience

  • ASN or higher required

  • Strong discharge planning and care coordination experience

  • Excellent communication, assessment, and critical thinking skills

  • Ability to collaborate effectively within multidisciplinary teams

  • Experience navigating insurance and authorization processes

Preferred Qualifications

  • Experience working with underserved or high-acuity patient populations

  • EPIC EMR experience

  • Familiarity with Careport or similar referral platforms

  • Schedule & Coverage

  • Monday-Friday, 8:00 AM - 4:30 PM

  • Occasional late days as needed

  • Every 5th weekend rotation

  • Employees working the weekend rotation receive the following Monday and Friday off

  • No on-call requirement

  • Approximately 50% holiday coverage (up to 2 holidays per contract)
