Registered Nurse II (Home Health)

What you’ll do

Complete Start of Care (SOC) within 48 hours of each qualifying visit and perform ongoing reassessments.

Deliver skilled nursing per physician orders and plan of care, updating and revising as needed.

Provide nursing services requiring professional judgment and specialized skills.

Document accurately and on time: clinical/progress notes, care conference summaries, OASIS assessments (SOC, ROC, Recert, Transfer, Discharge, SCIC); apply ICD-10 coding appropriately.

Educate and support patients and families on disease management, medications, and safety.

Coordinate care with physicians, therapists, social workers, and other members of the care team.

Lead and supervise LPNs and HHAs; conduct patient care conferences and required supervisory visits (per policy for LPN/HHA).

Participate in peer review, performance improvement, utilization review, and in-service programs.

Provide total patient care as needed and participate in call rotation (nights/weekends/holidays).

Support office-based clinical tasks when census is low.

What makes you a great fit

Required

Graduate of an accredited school of nursing; current RN license in practicing state(s).

4+ years of nursing experience.

Ability to exercise initiative and independent judgment in the field.

Reliable transportation to travel between patient homes.

Preferred

Prior Home Health or Acute Care experience.

Comfortable collaborating with an interdisciplinary team (physicians, patients, families, caregivers).

Work environment & physical requirements

Care delivered in patient homes with travel to and from visits; periodic time in the agency office.

High-risk exposure setting; adheres to infection control and safety protocols.

Ability to stand for extended periods; frequent bending and lifting; heavy lifting when necessary.

Reporting & supervisory structure

Reports to: Director of Clinical Services / Nursing Supervisor

Supervises: Licensed Practical Nurse (LPN), Home Health Aide (HHA)

Conducts required supervisory visits to patient homes per agency policy (bi-weekly for HHA; monthly for LPN) to assess care, relationships, and goal attainment.
