Job description

Nuvance Health is seeking a Nurse Practitioner to work in collaboration with our Medical Oncologist at Northern Dutchess Hospital in Rhinebeck, NY. The model of care is multi-disciplinary, integrated and disease site specific. This position will coordinate, oversee and provide advanced nursing care to medical oncology patients and functions as a Clinical Patient Navigator for this population.
Job Highlights
  • Works collaboratively with physicians and nurses and is responsible for providing comprehensive care to patients.
  • Conducts comprehensive and episodic heath history and physical assessment of patients.
  • Oversees treatment infusions and supportive care needs.
  • Writes orders within the limits of the practice agreement.
  • Maintains clinical documentation according to group practice.
  • Participates in clinical quality and research as needed.
  • Educates patients, families and caregivers on treatment plans.
  • Participates in learning opportunities.

Salary Range: $120,000- $170,000 yearly commensurate with experience
