Job description

Overview

Landmark Infusion is seeking a highly skilled TPN Pharmacist to join our growing home infusion nutrition support team. This role is ideal for a pharmacist experienced in a fast-paced clinical environment who can efficiently evaluate and transition short-term acute total parenteral nutrition (PN) patients discharging from the hospital into the home setting.

As a clinical leader in nutrition support, you will play a vital role in ensuring the safe, effective, and compliant delivery of PN therapies. This position requires strong clinical judgment, excellent communication skills, and the ability to collaborate across disciplines to coordinate timely, high-quality patient care.

Key Responsibilities

  • Review and verify PN and nutrition support orders for clinical appropriateness, stability, and accuracy
  • Serve as the subject matter expert in parenteral nutrition therapy and related metabolic complications
  • Conduct comprehensive clinical assessments, including laboratory interpretation and nutrient adjustments
  • Oversee sterile compounding, quality assurance, and dispensing of parenteral preparations in compliance with USP <797>, <800>, and regulatory standards
  • Develop and optimize PN formulations based on patient-specific clinical needs
  • Collaborate with physicians, dietitians, nursing staff, and business development teams to optimize outcomes and ensure smooth care transitions
  • Provide patient and caregiver education on PN administration, catheter care, and potential complications
  • Monitor patients longitudinally and make therapy adjustments based on clinical response and lab trends
  • Participate in on-call rotation or after-hours clinical support as needed
  • Support accreditation readiness (e.g., ACHC, Joint Commission) and maintain documentation standards
  • Assist in developing protocols, policies, and clinical pathways related to nutrition support
  • Mentor and provide guidance to pharmacy staff and support personnel
  • Contribute to quality improvement initiatives and patient safety programs

Qualifications

  • Doctor of Pharmacy (PharmD) or equivalent required
  • Active and unrestricted pharmacist license in the state of Texas
  • Ability to commute to our office in Irving, TX Monday through Friday
  • Minimum 2+ years of sterile compounding or home infusion experience
  • Minimum 3+ years of experience managing PN patients
  • Strong clinical expertise in nutrition support therapies and best practices
  • Board Certification in Nutrition Support Pharmacy (BCNSP) and/or Master’s degree in Nutrition preferred
  • Working knowledge of USP <797>, <800>, and home infusion regulatory requirements
  • Experience with EMR/clinical documentation systems and infusion pharmacy software
  • Excellent communication, critical thinking, and organizational skills
  • Ability to manage multiple priorities in a fast-paced, high-growth environment
