What You'll Do
As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team - not just a support role, but a trusted presence for patients and families. Your responsibilities include:
  • Providing personal care with dignity, comfort, and respect at the center of everything
  • Attending to patient needs and requests promptly and compassionately
  • Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed
  • Observing patients and reporting changes to the care team accurately and in a timely manner
  • Communicating with patients and families in a positive, ethical, and confidential manner
  • Maintaining thorough documentation in line with organizational standards
  • Upholding the hospice philosophy with professionalism and genuine compassion

What You Bring
  • Active CNA License or Certification in the state of employment
  • Possess and maintains current CPR certification
  • One (1) + year Hospice, home health or related organization experience preferred
  • Comfort and sensitivity working with terminally ill patients and their families
  • A calm, dependable presence under emotionally demanding circumstances
  • Strong attention to detail in observation and documentation
  • A valid driver's license and willingness to travel locally for patient visits

Salary Description
$19.00 - $23.00 an hour
