What Would You Be Doing?
  • Monitoring the ongoing continuous quality improvement process.
  • Supervising, orientating, and providing leadership to your field staff.
  • Coordinate cares with members of the interdisciplinary team, including but not limited to field staff, client, family members, physician(s), and other care providers as identified by the client / family.
  • Assist Administrator in implementing goals and objectives which are consistent with those of the organization.
  • Develop, implement and maintain integrated Plans of Care upon admission and not less frequent than 5 days of cert period.
  • Participates in monitoring and discussion of staffing needs to ensure optimal staff coverage in accordance with the physician's Plan of Care (POC).
Essential Functions:
  • Responsible for orienting direct reports who provide direct client care to ensure staff is competent in following the Plan of Care and providing continuous cares. Provide any additional training, as needed.
  • Coach, develop, and evaluate direct reports.
  • Responsible for the overall supervision and management of direct reports including, but not limited to distribution of workload and duties, performance management, professional development, monitoring responsibilities and deadlines, conducting one-on-ones, and providing conflict resolution when needed.
  • Responsible for being familiar with Agency policies and procedures, federal, state, and local laws, and regulations, and accepted professional standards and principles.
  • Interpret and transmit policies and procedures of the Agency to direct reports.
  • Coordinate cares with members of the interdisciplinary team as directed by the Administrator including the client, family members, nursing staff, the attending providers and other care providers.
  • Facilitate coordination of initial staffing as directed by the Administrator and Corporate Clinical Team, to ensure the safety and security of the client.
  • Support office goals and objectives which are consistent with those of the Agency.
  • Under the direction of the Administrator develop, implement, and maintain integrated Plans of Care upon admission and throughout duration of service period according to State and Federal guidelines and company policy.
  • Participate in monitoring and discussion of staffing needs to ensure optimal staff coverage in accordance with the Plan of Care (POC).
  • Assist in the interviewing process of new employees and makes recommendations for employment of individuals.
  • Provide 90 day and annual performance evaluations in a timely manner and no later than required by company policy, federal, state, or local laws.
  • Monitor the quality of care, client/family/client representative(s) (if any), satisfaction, and compliance with the Agency's policy and procedures.
  • Ensure proper maintenance of clinical records in compliance with local, state, and federal laws.
  • Monitor QAPI process and implement actions to ensure delivery of quality care.
  • Communicate any and all policy changes and/or updates to ensure that all Agency policies and procedures are implemented as written.
  • Advocate for client needs with providers, payers, DME vendors, and others as needed.
General Functions:
  • Participate in team meetings, Agency committees, goal initiatives, and other meetings or projects as directed.
  • Participate in the after-hours on-call rotation.
  • Serve as a CPR instructor and conduct CPR classes or checkoffs as needed or directed.
  • Network within the community to market the services the Agency provides.
  • Perform other duties and projects as requested.
What's Required?
  • Degree from an accredited school of nursing.
  • RN license without restrictions in state(s) of practice.
  • CPR certification required but may be obtained within 30-days of hire.
  • Prefer minimum two years' experience as a RN and one year of supervisory experience.
  • Must work well with minimal supervision.
  • Travel required to perform this job and may include occasional overnights.
  • Team player with a GREAT attitude.
  • Highly detail-oriented, organized, and efficient.
  • Ability to multi-task in a fast-paced environment and work independently with little supervision.
  • Excellent communication skills - both verbal and written.
  • Valid driver's license and proof of auto liability insurance that meets the state's minimum requirements.
  • Electronic Medical Record (EMR) experience is a plus.
Travel details:
  1. Duration of travel may be up to six hours one-way.
  2. Company car provided for travel.
  3. Company credit card for travel and marketing use.
  4. Travel arrangements booked through the Corporate Office Manager.
  5. Per diem rate for food and beverage for overnight or extended day travel.
