Role Summary
Full-Time | Monday–Friday | Eau Claire Service Area
Join a collaborative, patient-focused clinical team providing comprehensive acute and chronic care management for adult and geriatric patients in skilled nursing and senior care settings.
This role offers the opportunity to practice at the top of your license while partnering closely with supervising physicians and interdisciplinary care teams to improve patient outcomes and quality of life.
What You’ll Do
In this role, you will:
- Provide primary care for adult and geriatric patients with acute and chronic conditions
- Perform comprehensive assessments, establish diagnoses, and develop individualized treatment plans
- Prescribe medications and order/interpret diagnostic tests within scope of practice
- Coordinate care and clinical decision-making with facility teams and supervising physicians
- Maintain accurate, timely, and thorough clinical documentation
- Collaborate with specialty providers (e.g., wound care, psychiatry) to ensure integrated care delivery
- Communicate effectively with patients, families, and care teams regarding care plans and changes in condition
- Build strong, trust-based relationships with patients, families, and facility staff
- Support high-quality, consistent care across the Eau Claire service area
What You Bring
- Active Nurse Practitioner or Physician Assistant license in good standing
- Active DEA registration
- Current BLS certification
- Strong diagnostic and clinical reasoning skills
- Ability to work independently in a structured care environment
- Strong collaboration and communication skills
- Commitment to compassionate, patient-centered care
