As an LCPC serving within a community health clinic framework, you will have the opportunity to engage directly with diverse populations, addressing a range of mental health concerns in a supportive and collaborative atmosphere. You will utilize technology and electronic medical records (EMR) systems to facilitate client management and care coordination, ensuring that all interactions are documented accurately to support both client progress and strategic goals.

• Conduct comprehensive assessments to identify client needs, developing tailored treatment plans based on individual goals and circumstances.
• Maintain regular communication with a multidisciplinary team, including supervisors and direct care staff, to discuss client progress and collaboratively make necessary adjustments to treatment approaches.
• Provide a proposed schedule of appointments, home visits, and follow-up observations to track and monitor the advancement of each client’s care plan.
• Coordinate home visits, engaging in essential observational tasks, conducting chart reviews, and participating in staff interviews to ensure thorough and effective client support.
• Ensure that all aspects of the program plan are implemented accurately and effectively, making modifications as needed based on client feedback and observed outcomes.
• Participate actively in Interdisciplinary Team Meetings to provide insights and updates regarding client care while fostering a cohesive team environment.
• Maintain accurate and timely documentation of progress notes for both monthly and quarterly reviews, ensuring compliance with all regulatory and organizational standards.
• Facilitate and organize training sessions for team members, ensuring they are equipped to implement program plans effectively, including thorough understanding of documentation practices.
• Engage in follow-up assessments through interviews, home visits, and observational assessments to provide ongoing support tailored to individual needs.
• Act as a liaison with the public and mental health community, enhancing partnerships that will support the growth and development of community resources.

You will work with a broad spectrum of clientele, including both adults and youth, with varying needs from trauma and substance misuse to family dynamics. Helping clients navigate and overcome these challenges, your expertise will be instrumental in their journeys toward improved mental health and overall well-being.

• Master's or Doctoral degree in Counseling, Psychology, or a related discipline.
• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in Illinois.
• A minimum of two years of counseling experience in a clinical or community setting, demonstrating a strong foundation in mental health practices.

• Specialization in areas such as family therapy, couples therapy, trauma-informed care, or substance abuse counseling is highly desirable.
• A comprehensive understanding of ethical and legal standards is crucial, complemented by excellent communication skills that facilitate building rapport with diverse clients.
• Demonstrated ability to work effectively within a healthcare team, showcasing collaboration in care delivery.
• Proficiency in using EMR software and other technological tools relevant to client management.

• Annual salary range of USD 90,000 – 110,000, commensurate with experience and caseload.
