Key Responsibilities
  • Conduct comprehensive assessments to identify client needs, developing tailored treatment plans based on individual goals and circumstances.
  • Maintain regular communication with a multidisciplinary team, including supervisors and direct care staff, to discuss client progress and collaboratively make necessary adjustments to treatment approaches.
  • Provide a proposed schedule of appointments, home visits, and follow-up observations to track and monitor the advancement of each client’s care plan.
  • Coordinate home visits, engaging in essential observational tasks, conducting chart reviews, and participating in staff interviews to ensure thorough and effective client support.
  • Ensure that all aspects of the program plan are implemented accurately and effectively, making modifications as needed based on client feedback and observed outcomes.
  • Participate actively in Interdisciplinary Team Meetings to provide insights and updates regarding client care while fostering a cohesive team environment.
  • Maintain accurate and timely documentation of progress notes for both monthly and quarterly reviews, ensuring compliance with all regulatory and organizational standards.
  • Facilitate and organize training sessions for team members, ensuring they are equipped to implement program plans effectively, including thorough understanding of documentation practices.
  • Engage in follow-up assessments through interviews, home visits, and observational assessments to provide ongoing support tailored to individual needs.
  • Act as a liaison with the public and mental health community, enhancing partnerships that will support the growth and development of community resources.
Patient Population / Client Focus

You will work with a broad spectrum of clientele, including both adults and youth, with varying needs from trauma and substance misuse to family dynamics. Qualifications
  • Master's or Doctoral degree in Counseling, Psychology, or a related discipline.
