Hospice Palliative Nurse Practitioner (prn)

At Shalom Hospice, we strive to provide terminally ill patients and their loved ones with the support and care they require to live their lives comfortably. Our skilled hospice team is dedicated to helping our patients and their families maintain their dignity and quality of life. As a compassionate and reliable Per Diem Nurse Practitioner (NP), you will play a vital role in supporting our home-based hospice program in Nashville, TN.

Job Summary:

We are seeking a detail-oriented, responsive, and empathetic Per Diem Nurse Practitioner (NP) to complete Medicare-required Face-to-Face (F2F) visits for patients on service or entering their third or later benefit period. As a key member of our interdisciplinary team, you will conduct timely and compliant F2F hospice recertification visits, document clinical findings, collaborate with the IDG, and maintain schedule flexibility.

Responsibilities:

• Conduct timely and compliant F2F hospice recertification visits in patient homes, skilled nursing facilities, or assisted living settings
• Document clinical findings in accordance with Medicare hospice regulations and Shalom standards
• Collaborate with the interdisciplinary group (IDG) to support eligibility determinations and ongoing care planning
• Maintain schedule flexibility based on visit volume and regulatory timeframes

Qualifications:

• Active Nurse Practitioner license in Tennessee with prescriptive authority (if applicable)
• Board Certification in Adult-Gerontology, Family Practice, or related specialty
• At least 1 year of hospice or palliative care experience
• Strong documentation and time management skills
• Reliable transportation for community-based travel across the Nashville area
