Inpatient Palliative Care NP Opportunity

Expanding practice seeks an Inpatient Palliative Care NP to work in Findlay, Ohio! Join a collaborative, mission-driven Palliative Care team, working alongside experienced NPs, a Hospice Medical Director, and a Palliative Care Medical Director in a supportive, interdisciplinary environment.

Company: Nonprofit, integrated regional health system serving an eight-county region in Northwest Ohio. Over 4,000 patients served daily. One of only 45 members of the Mayo Clinic Care Network, giving physicians access to Mayo Clinic specialist consults at no cost to patients. Established Palliative Care program delivering inpatient care and partnering with a second hospital for inpatient and outpatient services. Providers follow patients throughout their hospital journey, focusing on symptom management, care coordination, and transitions of care. Collaborative, interdisciplinary team including hospitalists, specialists, nursing, case management, social work, and hospice teams.

Job/Practice Description: Inpatient Palliative Care NP

  • Full-time inpatient position: 75% at one hospital in Findlay, OH and 25% at a second hospital in Lima, OH
  • Mondayโ€“Friday schedule, 8:00 AMโ€“4:30/5:00 PM
  • Shared evening and weekend telephone call rotation
  • Perform inpatient palliative care consultations and follow-up visits
  • Manage symptoms for patients with serious, chronic, and life-limiting illnesses
  • Facilitate goals-of-care discussions and advance care planning with patients and families
  • Assist with discharge planning, including transitions to home-based palliative care or hospice
  • Provide inpatient rounding for hospice General Inpatient (GIP) patients at the second hospital as needed
  • Professionals with a passion for palliative care encouraged to apply

Requirements:

  • Certified Nurse Practitioner (Acute Care NP preferred; palliative care or hospice experience welcomed)
  • No experience needed!
  • Strong communication, interpersonal, and organizational skills
  • Comfortable navigating complex conversations related to serious illness, prognosis, and end-of-life planning
  • Ability to work independently while collaborating within an interdisciplinary team
