Position Overview
A well-established, mission-driven healthcare organization is seeking an experienced School-Based Nurse Practitioner (NP) to provide comprehensive healthcare services within a school setting in Brooklyn, NY. This role is ideal for a provider who is passionate about supporting the physical, emotional, and developmental well-being of students while collaborating closely with school administrators, educators, counselors, parents, and community partners.

Key Responsibilities
- Provide comprehensive primary and preventive healthcare services to students within the school setting
- Conduct physical examinations, health assessments, screenings, and wellness visits
- Diagnose and treat acute illnesses, minor injuries, and common pediatric/adolescent health conditions
- Manage chronic conditions such as asthma, diabetes, ADHD, and other ongoing health concerns
- Prescribe medications and treatments in accordance with New York State regulations
- Develop individualized care plans and coordinate care with families, primary care providers, specialists, and community resources
- Collaborate with school nurses, counselors, teachers, and administrators to support student health and well-being
- Provide health education to students, families, and school staff on topics such as nutrition, mental health, disease prevention, and healthy lifestyle practices
- Maintain accurate and timely electronic medical records (EMR) documentation
- Participate in health promotion initiatives, school wellness programs, and quality improvement activities
Qualifications
- Active and unrestricted Nurse Practitioner license in the State of New York
- Board Certification as a Family Nurse Practitioner (FNP) or Pediatric Nurse Practitioner (PNP)
- DEA license (or eligibility to obtain)
- Experience working in a school-based health center, pediatric practice, community health center, or similar setting preferred
- Experience collaborating with students, families, teachers, and school staff strongly preferred
- Strong clinical judgment and ability to work independently while functioning as part of a multidisciplinary team
- Bilingual (Spanish or other languages) is a plus but not required
