A well-established, mission-driven healthcare organization is seeking an experienced School-Based Nurse Practitioner (NP) to provide comprehensive healthcare services within a school setting in Brooklyn, NY. This role is ideal for a provider who is passionate about supporting the physical, emotional, and developmental well-being of students while collaborating closely with school administrators, educators, counselors, parents, and community partners.

The ideal candidate has experience working with pediatric and adolescent populations and understands the unique healthcare needs of students within an educational environment.

Key Responsibilities

  • Provide comprehensive primary and preventive healthcare services to students within the school setting
  • Conduct physical examinations, health assessments, screenings, and wellness visits
  • Diagnose and treat acute illnesses, minor injuries, and common pediatric/adolescent health conditions
  • Manage chronic conditions such as asthma, diabetes, ADHD, and other ongoing health concerns
  • Prescribe medications and treatments in accordance with New York State regulations
  • Develop individualized care plans and coordinate care with families, primary care providers, specialists, and community resources
  • Collaborate with school nurses, counselors, teachers, and administrators to support student health and well-being
  • Provide health education to students, families, and school staff on topics such as nutrition, mental health, disease prevention, and healthy lifestyle practices
  • Maintain accurate and timely electronic medical records (EMR) documentation
  • Participate in health promotion initiatives, school wellness programs, and quality improvement activities

Qualifications

  • Active and unrestricted Nurse Practitioner license in the State of New York
  • Board Certification as a Family Nurse Practitioner (FNP) or Pediatric Nurse Practitioner (PNP)
  • DEA license (or eligibility to obtain)
  • Experience working in a school-based health center, pediatric practice, community health center, or similar setting preferred
  • Experience collaborating with students, families, teachers, and school staff strongly preferred
  • Strong clinical judgment and ability to work independently while functioning as part of a multidisciplinary team
  • Bilingual (Spanish or other languages) is a plus but not required
