Description

The Nurse Navigator in Infusion serves as a clinical expert coordinating and administering patient care treatment regimens, research components and program outreach for patients diagnosed with cancer. The Navigator will assist patients and their families in managing through the vast array of services and programs offered by the hospital and the community to meet their health and wellness needs. The Nurse Navigator will provide personalized clinical and high touch care, advocating for the integration of whole person health of body, mind, and spirit. The Nurse Navigator will tailor services to the unique wellness needs of the patients served. The Nurse Navigator will participate in health screenings, provide patient education, and offer appropriate referrals. The Nurse Navigator will be responsible for direct patient clinical care along with cultivating relationships with patients and their families, physician partners, members of the healthcare team, and community partners. The Nurse Navigator in Infusion will be responsible for administration and management of treatment regimens, collection of data related to contacts and referrals as outlined by department manager. The Nurse Navigator will consistently demonstrate behaviors that model the PSJH core values and the hospital's mission statement

Required Qualifications:

  • Bachelor's Degree in Nursing
  • Upon hire: California Registered Nurse License
  • Within 30 days of hire: National Provider BLS - American Heart Association
  • Upon hire: Oncology Certified Nurse (OCN), Advanced Oncology Certified Nurse (AOCN), Advanced Oncology Certified Nurse Practitioner (AOCNP), Advanced Oncology Certified Clinical Nurse Specialist (AOCNS), Certified Breast Care Nurse (CBCN), or NCBC – Breast Patient Navigator Certification (or successful completion of one of these certifications within 1 year of hire)
  • Upon hire: Current ONS chemotherapy administration card
  • Five (5) or more years of experience as a Registered Nurse in a health care setting
  • Three (3) or more years of experience in oncology nursing

Preferred Qualifications:

  • Master's Degree in Nursing
