We are looking for a Nurse Practitioner ( NP ) for our Skilled Nursing Facility ( SNF ) in Maysville, KY who prides themselves on a positive attitude and dedication to providing top-notch care. We are looking for a dedicated and compassionate Nurse Practitioner ( NP ) for our Skilled Nursing Facility ( SNF ) in Maysville, KY. This is a full-time, in-person position, Monday through Friday daytime hours. No weekends, no nights, no on-call. The NP will serve as the primary care provider for assigned residents, working closely with the Medical Director, nursing staff, and the interdisciplinary care team.

Qualifications of the Nurse Practitioner ( NP ):

  • Active Kentucky NP license in good standing
  • National board certification (FNP or AGNP)
  • SNF, geriatric, or post-acute experience preferred
  • Strong clinical judgment and ability to work independently

Responsibilities of the Nurse Practitioner ( NP ):

  • Conduct comprehensive assessments, rounding visits, and follow-up care for residents
  • Diagnose and manage acute and chronic conditions common in the SNF population
  • Collaborate with the Medical Director, nursing staff, and care team on treatment plans
  • Document thoroughly and timely in the EMR
