  • The Nurse Practitioner will be responsible for rounding in hospitals and skilled nursing facilities, reviewing patients currently on our services and updating care plans accordingly. Under the direction of the Medical Director, the Nurse Practitioner will assess, review medication, create an appropriate care plan, provide and evaluate care. In addition, the Nurse Practitioner will assist with advanced directives and discuss goals of care and the disease process, progression, and grief process with patients and caregivers.
  • The Nurse Practitioner will work with the Director of Patient Care Services and Executive Director to provide education on Palliative and Hospice care within the community as well as agency staff regularly.

  • Current license from the state Board of Registered Nursing
  • Master's Degree from an accredited Nurse Practitioner's Program
  • National Certification as a Nurse Practitioner or certification eligible. Certification must be obtained within the first six (6) months of employment
  • A minimum of one (1) year Nurse Practitioner experience preferred
  • Previous Hospice experience preferred
  • Valid driver's license and reliable transportation
  • Able to pass criminal background check and national sex offender clearance
  • Must be able to perform patient care as needed
