Job description

Beloit Health System at Beloit Memorial Hospital is hiring a Diabetes Education Coordinator to join our team.

  • Shift: 1st, Varied
  • Schedule: 8a-4:30p
  • Hours per week: 40 hours per week
  • Benefits Status: Benefits Eligible
  • Department: Beloit Memorial Hospital Education

The Diabetes Education Coordinator is a registered nurse certified by the National Certification Board for Diabetes Educators. The coordinator must possess a broad range of knowledge of health and wellness and must have experience in care of people with diabetes and chronic diseases.

The Diabetes Education Coordinator will:

  • Serve as a liaison between the diabetes patient education advisory committee and the facility administration
  • Plan and participate in orientation sessions for new diabetes patient education personnel.
  • Provide and/or coordinate in-service education for diabetes patient education personnel
  • Design and carry out evaluations of the content and effectiveness of the diabetes patient education program
  • Coordinate ongoing and annual review of program resources
  • Provide overall coordination for the program and curriculum, including supervision of all instructional and clerical staff working for the diabetes patient education program and management of permanent patient education records.
  • Monitor and facilitate maintenance of DSMES team member and qualifications
  • Responsible for maintaining ADA recognition and participates in evaluation of DSME services effectiveness
  • Responsible for follow up with inpatient consults and outpatient referrals. Systematically and continuously collects and assesses data related to patient health status to develop, execute and evaluate the plan of care
  • Work with the education team on projects as assigned.
  • Other duties as assigned.

Job Requirements:

  • Graduate of accredited school of nursing. (BSN or MSN preferred)
  • Must obtain bachelor's degree within 4 years of hire.
  • State of Wisconsin or multistate nursing licensure required. (Must request WI licensure within 30 days); Current IL licensure as required by Northpointe job duties/assignments.
  • Certified Diabetic Educator certification through the National Certification Board for Diabetes Educators (CDCES) preferred at time of hire.
  • Minimum of three to five years' recent clinical experience preferred.
  • BLS certification within three months of hire.
  • Excellent oral and written communication skills are required.
  • Must be able to organize and prioritize multiple demands.
  • Maintain ethical conduct and keep confidential personal and medical information about patients.
  • Adhere to established health system and department policies, rules and regulations.
  • Participate in educational programs and continually seek for personal development.
