SummitWest Care | Mesa County, CO
Full-Time | Monday-Friday Day Schedule + On-Call Rotation
$35–$43/hour (DOE) + Mileage Reimbursement


A Sustainable Approach to Nursing

SummitWest Care is a nonprofit home health agency seeking a Registered Nurse (RN) to provide skilled, patient-centered care in the community. This is an in-person home health position based in Mesa County, CO, offering flexibility in how you structure your day, along with manageable caseloads and meaningful one-on-one care.


What We're Looking For

  • Active Registered Nurse (RN) License in State of Colorado (or active NLC compact license)
  • Valid driver's license, reliable transportation, and auto insurance
  • Strong assessment and critical thinking skills
  • Ability to work independently and manage a structured caseload
  • Clear, professional communication with patients and team members
  • Comfort with documentation and EMR systems (HCHB preferred)
  • Home health experience preferred, but not required

What You'll Do

  • Perform comprehensive patient assessments
  • Develop and update individualized plans of care
  • Provide skilled nursing care and appropriate interventions
  • Monitor patient progress and report changes to the care team
  • Educate patients and families on medications, treatments, and care plans
  • Coordinate care with LPNs, therapists, caregivers, and providers
  • Complete, timely, and accurate documentation in EMR
  • Participate in on-call rotation
