Charge Nurse (RN or LPN)
Job Description:
The Charge Nurse (Registered Nurse or Licensed Practical Nurse) is responsible for supervising and coordinating nursing care on an assigned shift within the nursing home facility. This role ensures residents receive high-quality, compassionate care in accordance with physician orders, facility policies, and regulatory requirements. The Charge Nurse provides clinical leadership to nursing staff, oversees daily care delivery, and serves as a primary contact for residents, families, and interdisciplinary team members. The Charge Nurse reports to the Director of Nursing.
Essential Job Duties:
  • Supervises and directs nursing staff on assigned shift, ensuring proper delivery of resident care.
  • Administers medications and treatments as prescribed, documenting accurately in resident records.
  • Monitors residents' conditions, reports changes to physicians and family members, and ensures timely interventions.
  • Provides guidance, training, and support to nursing assistants and other staff to promote best practices and adherence to care standards.
  • Ensures compliance with infection control, safety, and quality care standards.
  • Participates in resident assessments, care planning, and implementation of individualized care plans.
  • Responds promptly to emergencies, following established protocols.
  • Maintains open communication with residents, families, and interdisciplinary team members to promote satisfaction and resolve concerns.
  • Assists with orientation and continuing education of staff as directed by nursing leadership.
  • Ensures accurate shift-to-shift communication and completion of required documentation.

Job Knowledge & Skill:
  • Strong knowledge of nursing practices, long-term care regulations, and geriatric care principles.
  • Ability to lead, mentor, and motivate staff in a team-oriented environment.
  • Proficiency in medication administration, treatment protocols, and clinical documentation.
  • Excellent organizational, problem-solving, and critical-thinking skills.
  • Strong communication and interpersonal abilities for working with residents, families, and staff.
  • Ability to remain calm and make sound decisions in high-pressure situations.

Qualifications & Education:
  • Current and active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice required.
  • Graduate of an accredited nursing program.
  • Minimum of 1-2 years of clinical nursing experience in long-term care, skilled nursing, or related healthcare setting preferred.
  • Supervisory or leadership experience strongly preferred.
  • Candidate must demonstrate a moderate level of computer literacy in order to use and access our systems and tools within our facilities to complete online tasks and documentation.
  • Must be able to effectively communicate and interact professionally with staff and residents.

Physical Requirements:
  • Ability to stand, walk, bend, and move frequently throughout the shift.
  • Must be able to lift, push, pull, and move up to 50 pounds.
  • Ability to assist in transferring, repositioning, and providing care to residents.
  • Must have sufficient stamina and mobility to respond quickly to resident needs and emergencies.
  • Visual and auditory acuity required for monitoring residents and operating
