  • Participate in initiatives focused on reducing hospital admissions through proactive pulmonary and acute care management

  • Interpret and trend respiratory diagnostics, including ABGs, imaging, labs, pulse oximetry, and ventilatory data (as applicable)

  • Monitor patient response to treatments and intervene promptly to prevent respiratory decompensation

  • Coordinate care with pulmonologists, attending physicians, respiratory therapists, nursing staff, and facility leadership

  • Participate in pulmonary rounds and interdisciplinary care planning

  • Complete accurate and timely documentation in the EMR/EHR

  • Educate patients, families, and staff on respiratory conditions, CPAP/BiPAP use, tracheostomy care, and escalation protocols

  • Participate in initiatives to reduce respiratory-related hospital readmissions and emergency transfers

  • Ensure compliance with CMS, state regulations, and facility respiratory care and infection control policies
