Position Summary: Provides case management assistance through intakes, transportation and provides support in day-to-day functions of the safety wellbeing of UC’s. Assess relatives and/or sponsors at place of release to ensure their identification. Average travel could range between 5-10 travel events a month.
Essential Duties and Responsibilities: To perform this job successfully, an individual must be able to perform the following satisfactorily; other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

  • Actively participate in safety and service-related resources development prior to UC discharge.
  • Provide transportation support to assigned case management team.
  • Escort children, verify identification of approved sponsor and release custody of children to approved sponsor.
  • Prepare and maintain accurate documentation of reunification of children.
  • Attending scheduled training related to positions.
  • Provide day-to-day coverage within agency service facility.
  • Adhere to Child Abuse and reporting requirements.
  • Must be relied upon regarding task completion and meeting deadlines without sacrificing accuracy, work quality, or customer service satisfaction.
  • Perform other duties as assigned.
