Provide skilled in-home nursing care for low- to high-acuity patients Complete patient assessments and follow individualized care plans Administer medications and treatments as prescribed Collaborate with families and interdisciplinary team members Accurately document care and report changes to the RN Clinical Supervisor/Site Director Safely assist with lifting, positioning, and transferring patients
