Job description

Compensation Range: $16/hr - $19/hr commensurate with experience
Benefits:
We have three comprehensive healthcare plans to choose from based on your priorities and budget. Housing Works covers most of the plan; you pay a portion, based on your salary. Staff begins accruing PTO immediately for a total of up to 30 days earned in the first year. We offer employees an educational benefit. This money is available for tuition loan reimbursement, tuition costs, and text books.

Navegacion de participantes en necesidad de servicios y tratamiento de Hepatitis-C en varias regiones del sector este de la isla.
Responsibilities:
  • Identificar canales de captación y reclutamiento
  • Realizar actividades de divulgación dirigidas a personas de la población objetivo
  • Proporcionar intervenciones basadas en la evidencia como requisito del proyecto
  • Conectar a clientes con VHC y VIH con servicios de atención primaria
  • Conectar a los clientes con tratamientos para el VHC, el VIH y el trastorno por consumo de sustancias, según sea necesario
  • Proporcionar referencias a servicios de apoyo, según sea necesario
  • Brindar apoyo continuo para fomentar la retención en los servicios
  • Recopilar e informar datos sobre prácticas basadas en la evidencia
  • Realizar entrevistas y evaluaciones iniciales
  • Realizar evaluaciones de riesgo
  • Brindar asistencia en acompañamiento, transporte, servicios de seguimiento y apoyo
  • Colaborar en la coordinación de acuerdos de colaboración con otras organizaciones y socios comunitarios
  • Representar al programa y a la agencia en diferentes foros
  • Preparar informes mensuales y planes de trabajo
  • Colaborar en el desarrollo de capacitaciones educativas para diferentes organizaciones o grupos poblacionales
  • Garantizar la calidad y precisión de la documentación en los registros de los participantes

Requirements:
  • Diploma de Educación General (GED) o superior (se tendrá en cuenta la experiencia laboral en el ámbito de los servicios sociales).
  • Experiencia con programas dirigidos a poblaciones marginadas: VIH/VHC, consumo de sustancias y/o trastornos concurrentes en entornos de atención comunitaria, personas sin hogar, trabajadoras sexuales, entre otras.
  • Experiencia en la recopilación y presentación de datos.
  • Dominio de diversos programas informáticos.
  • Gran meticulosidad en el mantenimiento de registros y el cumplimiento de protocolos de confidencialidad.
  • Capacidad para trabajar en equipo.
  • Se valorará la experiencia en atención comunitaria no tradicional.
  • Vehículo propio y licencia de conducir vigente.
