Under clinical practice guidance of the Supervisor, the QMHP / Social Worker assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes.

Lexington County Detention Center
Night Shift 3:00pm - 11:30pm
Eligible for Shift Differential
Assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes. This role completes an assessment on the patient and caregivers throughout the hospitalization and outpatient follow-up as needed. This position identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. The QMHP / Social Worker (MSW) bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.
Education: Master's degree in a social work or social welfare program accredited by the Council on Social Work Education. Credentials: MSW licensure by the South Carolina Board of Social Work Examiners. Certification in case management preferred.
